Transitions Hospice is hiring a full-time RN to provide compassionate hospice care across Franklin County and surrounding areas. The role involves 8-hour weekday shifts with on-call responsibilities, building and updating care plans with physicians, and delivering comprehensive in-home care.

The ideal candidate has an active RN license, a nursing degree, and at least 1 year of hospice or home health experience, with strong communication and clinical judgment for independent field work.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.

Responsabilités

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate and timely.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.

Pay Range: $75,000 - $85,000 an hour
