Job summary

Transitions Hospice is seeking an RN Case Manager to be the clinical anchor for patients and families in Franklin County and surrounding areas. You will coordinate care, guide the interdisciplinary team, and respond when it matters most, building personalized care plans with physicians and care teams.

Your on-site schedule is Mon-Fri, 8-hour shifts with on-call responsibilities; requires an active Ohio RN license and hospice/home health experience preferred, along with strong communication and

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year hospice/home health experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand patient needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.

What You'll Do
