Job summary

Transitions Hospice is seeking an RN Case Manager to anchor patient care in Ottawa, Lasalle and surrounding areas. The role coordinates care, leads the interdisciplinary team, and participates in on-call rotation.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year Hospice, home health, or related experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.

Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Salary Range: $80,000 - $90,000 / year

What You'll Do
