Job summary

Transitions Hospice is seeking an RN Case Manager in Ottawa, IL to coordinate care and guide the interdisciplinary team. You will be the clinical anchor for patients and families, aligning plans with physicians and care teams to ensure compassionate, comprehensive end-of-life care.

The role requires an active RN license in Illinois, at least 1 year of hospice or home health experience, and strong clinical judgment for independent field work. Competitive salary with full benefits offered.

Qualifications

• Graduate of an accredited school of Nursing.
• 1+ year hospice, home health, or related organization experience preferred.

Responsibilities

• Conduct comprehensive, ongoing assessments of patients' evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate and timely.
• Serve as communication bridge between families, physicians, and health partners.
• Educate patients and caregivers to keep them informed and confident.
• Participate in on-call rotation per team policy.

Schedule: 8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Salary Range: $80,000 - $90,000 / year

What You'll Do
