Transitions Hospice in Illinois is seeking an RN Case Manager to serve as the clinical anchor for patients and families, coordinating care and guiding the interdisciplinary team. The role offers competitive pay and comprehensive benefits.

You will conduct assessments, develop care plans, and ensure accurate documentation while engaging with families and physicians to deliver patient-centered hospice care.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• 1+ year hospice, home health, or related experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers to ensure confidence and support.
• Participate in on-call rotation per team policy.

Coverage Territory: Sangamon, Morgan, Cass, Menard, Mason and surrounding counties

Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Salary Range: $75,000 - $85,000 a year
