Job summary

Transitions Hospice in Illinois is seeking an experienced RN Case Manager to serve as the clinical anchor for patients and families, coordinating care and guiding the interdisciplinary team. Your day-to-day includes conducting comprehensive assessments, building personalized care plans, administering medications, keeping documentation accurate, and serving as a communication bridge across care teams.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.

Coverage Territory: Sangamon, Morgan, Cass, Menard, Mason and surrounding counties

Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Salary Range: $75,000 - $85,000 a year
