Job summary

Transitions Hospice - Illinois in Buffalo Grove is seeking an RN Case Manager to coordinate patient care, guide the interdisciplinary team, and ensure comfort and dignity at end of life.

You will conduct assessments, develop care plans with physicians, administer orders, and maintain accurate documentation while supporting families and on-call rotation as required.

Qualifications

• RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year Hospice or home health experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication across care teams and families.

Responsibilities

• Coordinate care for patients and guide the interdisciplinary team.
• Build and adjust personalized care plans with physicians.
• Administer medications and treatments per orders.
• Maintain accurate, timely clinical documentation.
• Serve as the communication bridge between families, physicians, and community partners.
• Educate patients and caregivers to provide confidence and support.
• Participate in on-call rotation per team policy.

Buffalo Grove, Northbrook, Des Plaines, Niles, Elk Grove (Cook County/Lake County) and surrounding areas

Schedule:

8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation (on-call and rotating holiday requirements)

Pay Range:

$85,000 - $95,000 a year

What You'll Do

As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy
