Job summary

Transitions Hospice in Oak Park, IL seeks an RN Case Manager to coordinate care for patients and families in the home setting, leading the interdisciplinary team and guiding care plans with physicians to ensure compassionate, patient-centered support.

This full-time role requires an active IL RN license and 1+ year hospice experience; strong communication and independent work are essential. On-call and weekend rotation are included with a competitive salary and benefits.

Qualifications

• Active RN license in state of employment.
• Graduate of an accredited School of Nursing.
• 1+ year Hospice, home health or related experience preferred.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments of patients and families.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers to foster confidence and support.
• Participate in on-call rotation per team policy.

Coverage Territory: Oak Park, Cicero, North side Chicago and surrounding areas

Schedule: 8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotation (on-call and rotating holiday requirements)

Pay Range: $85,000 - $95,000 a year

What You'll Do

As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:
