Job summary

Transitions Hospice - Illinois seeks a dedicated RN to provide compassionate, at-home hospice care in Marshall and surrounding areas. The role offers a competitive salary range and a schedule featuring 8 AM-8 PM with 4-day and 3-day weeks in rotation.

You will conduct comprehensive assessments, develop care plans with physicians and care teams, administer medications, maintain documentation, and bridge communication between families and providers.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.
