Job summary

TRANSITIONS HOSPICE ILLINOIS II LLC is seeking an RN Case Manager to serve in Bloomington and surrounding areas. You will coordinate care, guide the interdisciplinary team, and ensure 24/7 support within home-based hospice services.

The role requires an active RN license in the state of employment, and prior hospice or home-health experience is preferred. The schedule features 8 AM–8 PM shifts with rotating 4-day and 3-day weeks, including weekends.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation.
• Serve as the communication bridge between families, physicians, and partners.
• Educate patients and caregivers to ensure confidence and support.
• Participate in on-call rotation per team policy.

What You'll Do
