Job summary

Transitions Hospice is seeking an RN Case Manager to serve as the clinical anchor for patients and families in Indianapolis and surrounding areas. The role coordinates care, guides the interdisciplinary team, and responds when it matters most.

You will conduct assessments, develop personalized care plans, administer medications per orders, and maintain accurate documentation. A valid RN license and nursing degree are required, with hospice/home health experience preferred.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.

Responsibilities

• Conduct comprehensive, ongoing assessments of patients to understand evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation.
• Serve as communication bridge between families, physicians, and community partners.
• Educate patients and caregivers to foster confidence and support.
• Participate in on-call rotation per team policy.

What You’ll Do

Requirements

What You Bring

• Active RN license in the state of employment
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care
• Strong clinical judgment and the ability to work independently in the field
• Excellent communication skills across care teams and families
