Job summary

Transitions Hospice - Michigan is seeking an experienced RN Case Manager to serve Bay City and surrounding counties, coordinating care and guiding the interdisciplinary team in a patient-centered hospice program.

You will conduct comprehensive assessments, develop personalized care plans, administer physician-ordered treatments, document diligently, educate families, and participate in on-call rotation, ensuring compassionate, high-quality end-of-life care.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year Hospice/home health or related experience preferred.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication across care teams and families.

Responsibilities

• Conduct comprehensive ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely, and meaningful clinical documentation.
• Serve as the communication bridge between families, physicians, and community partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.

Salary Range: $40.00 - $46.00 an hour

What You'll Do
