Transitions Hospice in Illinois is seeking an RN Case Manager to be the clinical anchor for patients and families, coordinating care and guiding the interdisciplinary team. You will assess, develop and adjust care plans, administer medications per orders, and ensure accurate documentation.
This role requires an active RN license in the state, at least one year of hospice or home health experience, and strong communication across teams.

Rockford, Freeport, Belvidere and surrounding areas
8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation
