Job summary

Transitions Hospice in Bloomington, IL seeks an experienced RN Case Manager to coordinate patient care and lead the interdisciplinary team across a 24/7 service area. You will manage assessments, care plans, medications, and documentation while serving families with compassion.

The role covers Bloomington and surrounding areas, with 8 AM–8 PM shifts on a rotating 4-day/3-day schedule including weekends. Strong nursing judgment and clear communication are essential."

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• One (1) year Hospice or home health experience preferred.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keeping clinical documentation accurate, timely, and meaningful.
• Serving as the communication bridge between families, physicians, and community health partners.
• Educating patients and caregivers so they feel confident and supported.
• Participating in on-call rotation per team policy.
