Job summary

Pathways Hospice is seeking a licensed registered nurse to join our growing team delivering high-quality hospice nursing care in Houston's Northwest, Southwest and Southeast areas. This PRN and On-Call role supports patient-centered care, collaborates with physicians and therapists, and plans and directs home-based services.

The ideal candidate holds a TX RN license, hospice experience, and knowledge of home health regulations; proficiency with HCHB EMR is a plus.

Qualifications

• RN license valid in Texas.
• Hospice experience (1 year minimum).
• Knowledge of home health regulations and compliance.
• Experience with Home Care Home Base (HCHB) EMR is a plus.
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients
