Job summary

Pathways Hospice is seeking a dedicated Hospice RN Case Manager to join our team in the Houston area. This full-time role oversees the nursing care team, collaborates with physicians and therapists, and directs home care services to deliver comprehensive patient care.

Weekend and on-call rotations are required to ensure consistent patient support. Qualified applicants will hold a Texas RN license, have hospice experience, and be proficient with HCHB EMR.

Qualifications

• Valid RN license in the State of Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients
• If working the weekend-will receive the following Monday off (Admissions Included)
• Must be willing to participate in an On Call Rotation (Weekly Rotation)
