Job summary

Pathways Hospice is seeking a licensed registered nurse to join our growing team delivering high-quality hospice nursing care in Houston's Northwest, Southwest and Southeast areas. This PRN and On-Call role supports patient-centered care, collaborates with physicians and therapists, and plans and directs home-based services.

The ideal candidate holds a TX RN license, hospice experience, and knowledge of home health regulations; proficiency with HCHB EMR is a plus.

Qualifications

RN license valid in Texas.
Hospice experience (1 year minimum).
Knowledge of home health regulations and compliance.
Experience with Home Care Home Base (HCHB) EMR is a plus.
Excellent communication and people skills.
Ability to work independently and as part of a team.

Responsibilities

Oversee nursing care team.
Collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
Plan, organize, and direct home care services.
Conduct assessments of patient care needs and develop care plans.
Educate patients and their families on managing health conditions, medication regimens, and any necessary lifestyle changes.
Prepare timely, complete, and accurate clinical notes.
Night and Weekends are required per hospice needs of patients
