Job summary

Frontpoint Health in Houston, TX is seeking a Hospice - PRN Registered Nurse (RN) to travel to patient homes in the North Houston area, delivering skilled nursing care and hospice support to maintain patient independence and comfort.

The role requires current Texas RN licensure and hospice experience. You will perform assessments, manage wound care and IVs, document visits within 24 hours, and collaborate with the care team on the plan of care.

Qualifications

• Current Texas State license as a Registered Nurse.
• 2+ years experience as a RN in Hospice.
• Current CPR certification.
• Reliable transportation.
• Texas Drivers License and Auto Liability Insurance.
• Current TB Skin Test.
• EMR experience (WellSky Consolo preferred).

Responsibilities

• Case management and supervised visits.
• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patient’s condition at each visit (vital signs).
• Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing healing progress).
• Provide proper patient education for home health care procedures and strategies.
• Responsible for the overall clinical record maintenance to include visit notes, comprehensive assessments, including OASIS, medication profile, orders/plan submitted timely.
• Maintain confidentiality of patient PHI through HIPAA law.
• Keep the patient comfortable through palliative care.

As a Hospice Nurse or RN, you will be responsible for traveling to several homes a day to administer healthcare services and help patients maintain their independence. Duties may include administering at home IVs, changing dressings, cleaning wounds, and foley/catheter insertions.
