Pathways Hospice seeks a dedicated Hospice Clinical Supervisor RN to oversee planning, coordination, and delivery of quality interdisciplinary hospice care services at our Main Branch (77081).

The role ensures compliance with federal/state standards and clinical best practices while supporting clinicians, maintaining our quality model, and guiding staff development.

Qualifications

• RN licensed in Texas.
• Experience supervising hospice teams.
• Strong communication and teaching abilities.

Responsibilities

• Supervises, monitors, and ensures competencies and performance of assigned hospice care team(s).
• Supervises ongoing case management of all patients ensuring accurate evaluation and treatment plans and adherence to the quality model.
• Reviews electronically submitted documentation and provides feedback to clinicians for deficiencies or corrections to ensure medical records reflect compliance.
• Reviews agency data, participates in quality and outcome improvement activities.
• Reviews patient schedules to ensure coordination of care and efficiency.
• Educates and mentors the home health care team regarding documentation, standards of care, and compliance.

Responsibilities

• Supervises, monitors, and ensures competencies and performance of assigned hospice care team(s)
• Supervises ongoing case management of all patients ensuring an accurate evaluation and treatment plan, maintaining our quality model, and utilizing services appropriately to achieve optimal outcomes.
• Reviews electronically submitted documentation and provides feedback to clinicians regarding deficiencies or OASIS corrections (with feedback from Quality Review Specialist) to ensure the medical records reflect compliance with medical necessity, homebound status, visit utilization, and documentation of individualized patient assessment and transition (discharge) planning.
• Reviews agency assigned team outcome data, participates in and facilitates improvement activities for agency quality and outcome performance.
• Reviews patient schedules to ensure appropriate coordination of care and clinical needs and ensures efficiency of clinical associates.
• Serves as an educator and mentor for assigned home health care team regarding home health documentation, including addressing deficiencies in clinical documentation, inconsistencies, clinical standards of care, and compliance.

Must-have's To Be Considered For This Position

• 2+ years experience, as Hospice Registered Nurse (RN) Supervisor
• Supervisory Experience in Hospice
• Can reliably commute to the Branch/ Work out of Branch
• RN in the State of Texas
• Experience with HCHB EMR (preferred)
• Knowledge of Home Health regulations and general healthcare compliance
• Proficient in all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
