Job summary

Pathways Hospice in Houston is seeking a dedicated RN to support hospice care across the Northwest, Southwest & Southeast Houston areas. The role is PRN and On Call, with night and weekend requirements to meet patient needs and ensure continuous care.

You will lead nursing teams, coordinate with physicians and therapists, and plan home care services while educating families on regimens and lifestyle adjustments. Strong communication and teamwork are essential for quality patient outcomes.

Qualifications

• Valid RN license in the State of Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients
