Job summary

Empath Health in Naples seeks a compassionate Home Health RN to provide in-home skilled nursing across Collier County. You will perform comprehensive assessments, including OASIS, and implement personalized care plans for patients in their homes.

The role offers a Monday-to-Friday schedule with 40 hours and on-call rotations on weekends and holidays; weekend coverage is required. Activation of the care team and patient education are essential to support full life care at home.

Qualifications

• RN license in Florida (active)
• Minimum 1 year direct patient care experience; home health RN preferred
• Graduate of an approved nursing program
• Weekend and holiday on-call rotations required
• Demonstrated assessment, critical thinking, and patient education skills

Responsibilities

• Perform comprehensive patient assessments including OASIS
• Develop, initiate and revise plans of care
• Deliver skilled nursing services per plan of care
