Job summary

Empath Home Health is seeking a compassionate Home Health RN to provide patient-centered care in Naples and surrounding areas. The role includes OASIS assessments, care planning, and education for patients and families, with on-call rotations on weekends and holidays.

Empath Home Health, a member of Empath Health is seeking a compassionate Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes throughout the Naples area. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits in Collier county (including Naples, Bonita Springs, Marco Island, Immokalee, Ave Maria, and surrounding areas).

Schedule: M-F 40 hours. Required weekend & holiday on-call rotations.

What You’ll Need

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
