Job summary

Empath Home Health, part of Empath Health, seeks a compassionate Home Health RN to provide care to patients in their homes across Charlotte County and surrounding areas. Schedule is M-F, 40 hours with weekend/holiday on-call rotations.

Requirements include a Florida RN license, with a BSN or MSN preferred, and 1 year of direct patient care; home health experience is strongly preferred. Join a mission-driven team delivering Full Life Care and CEU support.

Qualifications

• Graduate of an approved nursing program with an active Florida RN license.
• Prefers BSN or MSN degree.
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
