The Home Health Clinical Coordinator is responsible for ensuring a smooth transition from hospital to home by coordinating with facility staff, patients, and families to facilitate timely and appropriate admissions. The Home Health Clinical Coordinator will will work in the Naples, FL area possess a valid clinical FL license. What You'll Do
