Job summary

Empath Health is seeking a compassionate Home Health RN to join our Home Health team serving Pinellas County (Clearwater, St. Petersburg, Palm Harbor, Gulfport, and surrounding areas).

You will perform comprehensive patient assessments, develop care plans, deliver skilled nursing care, educate patients and families, and collaborate with the interdisciplinary team to ensure holistic, Full Life Care at home.

Qualifications

• Graduate of an approved nursing program with an active Florida RN license.
• BSN or MSN degree preferred.
• Minimum of 1 year direct patient care; home health RN preferred.
• Willingness to participate in weekend/holiday on-call rotations.
• Strong assessment and patient education skills.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments per plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure holistic care.

Empath Home Health, a member of Empath Health is seeking a compassionate Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes in Pinellas county (including Clearwater, St. Petersburg, Palm Harbor, Gulfport,, and surrounding areas).

Locations: Home health visits in Pinellas county (including Clearwater, St. Petersburg, Palm Harbor, Gulfport, and surrounding areas).

Schedule: M-F 40 hours with weekend/holiday on-call rotations required.

What You’ll Need

• Minimum of 1 year of direct patient care experience; prior home health RN experience preferred
• Strong assessment, critical thinking, and patient education skills
