Job summary

Pride Health is seeking a Registered Nurse - Home Health to support a client facility in Wallingford, CT. This local contract role runs Monday through Friday, 9:00 AM to 5:00 PM, with coverage in Groton and surrounding towns.

The position requires an active RN license, CPR certification, and 1+ year of home health experience; Homecare Homebase (HCHB) experience is a plus. Mileage reimbursement is provided, with a pay range of $50–$56 per hour (W-2).

Qualifications

• 1+ years of Home Health experience is required.
• Current RN license in the appropriate state.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Provide skilled nursing care in the home setting with comprehensive assessments and care plans.
• Educate patients and caregivers on disease management, medications, infection prevention, equipment use, and self-care.
• Maintain accurate clinical documentation and communicate changes to the care team, adhering to policies and nursing standards.

Pride Health is hiring a Registered Nurse - Home Health to support our client's healthcarefacility in Wallingford, CT.

Job Details

Schedule:Monday-Friday | 9:00 AM-5:00 PM

Location:Wallingford, CT

Coverage Area: Groton, CT and Surrounding towns

Job Type: Local contract

Contract Length:14+ Weeks (Possibility to Extend)

Local Pay Range:$50.00 - $56.00/hour (W-2) Plus Mileage reimbursement

Mileage Reimbursement Rate: $0.50/ mile

Duties

• Provide skilled nursing care to patients in the home setting, performing comprehensive assessments, developing individualized care plans, and monitoring patient progress in accordance with physician orders.
• Educate patients and caregivers on disease management, medication administration, infection prevention, medical equipment uses, and self-care techniques to improve health outcomes and promote independence.
• Maintain accurate clinical documentation, communicate changes in patient condition to the interdisciplinary healthcare team, and ensure compliance with agency policies, Medicare guidelines, and professional nursing standards.

Requirements

• License(s): Current and unrestricted RN licensure in state of practice
• Certification(s): Current CPR Certification or ability to complete within 90 days of hire
• Years of experience:1+ years of Home Health experience and Homecare Homebase (HCHB) is a plus
