Responsibilities

• Ensure all activities align with Elara Caring’s vision and leadership directives.
• Assess assigned caseload of home health care patients and families to identify needs.
• Maintain patient and staff privacy and confidentiality pursuant to HIPAA.
• Assess patient needs and develop, implement, and evaluate care plans.
• Communicate significant findings and changes in health condition to relevant team members.
• Revise care plans as needed and coordinate with the healthcare team.

What is Required?

• Current, unrestricted state RN license
• 1+ year experience in a clinical care setting as a nurse
• Willingness to travel within branch/office coverage area
• Dependable vehicle, valid driver’s license, and current auto insurance
• Ability and willingness to travel 50%
• Associates Degree or Bachelor’s Degree in Nursing preferred
• Previous Home Health or Hospice experience preferred
• Experience with HomeCare HomeBase preferred

You will report to the Clinical Manager or Clinical Supervisor.
