The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies. Responsibilities include:

• Coordinating care with other healthcare professionals
• Planning and exchanging information to ensure continuity of care
• Performing initial and comprehensive nursing evaluations
• Developing and implementing care plans within state guidelines
• Documenting care accurately and timely
• Conducting assessments and developing appropriate care plans
• Ensuring patients meet home health eligibility and medical necessity guidelines

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to dependable transportation
