The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in accordance with laws, regulations, and agency policies.

• Coordinates care plans with healthcare professionals and ensures continuity of care by sharing information with physicians, agency staff, patients, families, and community resources within a Point of Care setting.
• Conducts initial and comprehensive nursing evaluations, develops care plans within state guidelines, and documents accurately and timely in collaboration with physicians.
• Performs patient assessments and develops appropriate care plans.
• Ensures patients meet home health eligibility and medical necessity criteria as defined by payers.

License Requirements:

• Current RN license in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation or public transit
