The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.

Responsibilities include coordinating the total plan of care with other healthcare professionals, conducting initial and comprehensive nursing evaluations, developing and implementing the plan of care, performing assessments, ensuring patients meet home health eligibility and medical necessity guidelines, and maintaining accurate documentation.

License Requirements

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation
