Job summary

Pathways Hospice is seeking a full-time RN Case Manager in Texas to oversee the nursing care team and coordinate home health services. You will collaborate with physicians, therapists, and healthcare professionals to plan and direct patient care, educate families, and ensure accurate clinical documentation.

The role requires an active Texas RN license, hospice experience, and strong communication. Night/weekend shifts and weekly on-call rotation are part of the schedule, with weekend work

Qualifications

• Valid RN license in Texas.
• Hospice Experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR.
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients.
• If working the weekend-will receive the following Monday off (Admissions Included).
• Must be willing to participate in an On Call Rotation (Weekly Rotation).
