Job summary

Pathways Hospice is seeking an experienced RN Case Manager to oversee the nursing care team and coordinate patient care across our Northwest, Southwest & Southeast territory. You will collaborate with physicians and other healthcare professionals to deliver cohesive, high-quality hospice services and manage care plans for patients and families.

Requirements include a Texas RN license, at least 1 year of hospice experience, and proficiency with Home Care Home Base (HCHB) EMR.

Qualifications

Valid RN license in Texas.
Hospice experience of at least 1 year.
Knowledge of home health regulations and compliance.
Experience with Home Care Home Base (HCHB) EMR.
Excellent communication and people skills.
Ability to work independently and as part of a team.

Responsibilities

Oversee nursing care team.
Collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive patient care.
Plan, organize, and direct home care services.
Conduct assessments of patient care needs and develop care plans.
Educate patients and families on managing health conditions, medications, and lifestyle changes.
Prepare timely, complete, and accurate clinical notes.
Night and Weekends are required per hospice needs of patients.
If working the weekend-will receive the following Monday off (Admissions Included)
Must be willing to participate in an On Call Rotation (Weekly Rotation)
