Job summary

Pathways Hospice is seeking an experienced RN Case Manager to oversee the nursing care team and coordinate patient care across our Northwest, Southwest & Southeast territory. You will collaborate with physicians and other healthcare professionals to deliver cohesive, high-quality hospice services and manage care plans for patients and families.

Requirements include a Texas RN license, at least 1 year of hospice experience, and proficiency with Home Care Home Base (HCHB) EMR.

Qualifications

• Valid RN license in Texas.
• Hospice experience of at least 1 year.
• Knowledge of home health regulations and compliance.
• Experience with Home Care Home Base (HCHB) EMR.
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and families on managing health conditions, medications, and lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients.
• If working the weekend-will receive the following Monday off (Admissions Included)
• Must be willing to participate in an On Call Rotation (Weekly Rotation)
