Job summary

Pathways Hospice is seeking a dedicated Hospice RN Case Manager to join our team in the Houston area. You will oversee the nursing care team and coordinate home-based services for patients and families.

The role requires a Texas RN license and at least 1 year of hospice experience, with strong communication and collaboration skills to work with physicians and support staff. Nights/weekends and on-call rotation may be required. Competitive benefits and mileage reimbursement are offered.

Qualifications

• Active RN license in Texas.
• Hospice experience of 1 year or more.
• Knowledge of home health regulations and healthcare compliance.
• Experience using HCHB EMR software.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Collaborate with physicians, therapists and other healthcare professionals to ensure cohesive patient care.
• Plan, organize and direct home care services.
• Assess patient care needs and develop care plans.
• Educate patients and families on managing health conditions and medications.
• Prepare timely, complete clinical notes.
• Night and weekend shifts are required per patient needs; on-call rotation weekly.

Hospice RN Case Manager

Shift Available: Full Time

Territory for Coverage: Northwest, Southwest & Southeast Houston

Responsibilities:

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients
• If working the weekend-will receive the following Monday off (Admissions Included)
• Must be willing to participate in an On Call Rotation (Weekly Rotation)

Qualifications:

• Valid RN license in the State of Texas.
• Hospice Experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.
