JourneyCare Hospice is seeking an RN Case Manager to join our team in Arlington Heights, IL. You will manage a patient and family caseload, coordinate care across the interdisciplinary team, and ensure timely assessments and care planning. The role requires an active Illinois RN license, CPR/First Aid certifications, and a valid driver's license. Weekend coverage is on a rotating schedule, with strong emphasis on quality patient care

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess CPR and First Aid certifications.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation.
• Obtain data from patient physical, psychological, social, and spiritual factors that may impact needs and coordinate with the interdisciplinary team.
• Initiate communication with attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the care plan.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records, charts, and progress notes.

HOURS: Monday-Friday 8:30am-5pm and One Weekend Every 12th Week (Saturday and Sunday)

TERRITORY: Arlington Heights, IL

Salary: $80,000-$97,000 Annually - Offer Based on Years of Experience

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
