Job summary

Aleca Hospice in Arizona is seeking a dedicated Registered Nurse to plan, organize and direct hospice care with emphasis on community health education and holistic patient support. You will conduct in-home visits, develop care plans, and collaborate with physicians and the care team to ensure compassionate care.

Qualifications

• Active RN license through the State Board of Nursing.
• Community health/hospice experience preferred but not required.
• Demonstrates strong verbal and written communication, and organization skills.

Responsibilities

• Completing hospice evaluations as requested; admissions where appropriate.
• Conducting in-home visits; assess patient condition and educate patient and family.
• Developing care plans based on diagnosis and needs; coordinate with care team.
• Administering medications and treatments as prescribed by physician.
• Communicating patient needs and changes with the team.

Responsibilities

• Completing hospice evaluations as requested (admission if comfortable and training would be provided)


• Conducting in home visits as assigned; assessing the patient’s condition, and teaching the patient and family


• Developing care plans based on diagnosis and needs


• Administering medications and treatments as prescribed by physician


• Communicating patient needs and changes with the team as necessary

Qualifications

• Must hold a current, valid RN license through the State Board of Nursing


• Community health/hospice is preferred but willing to train the right candidate.


• Complies with accepted professional standards and practice.


• Demonstrates good verbal and written communication, and organization skills.


• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.


• Possesses and maintains current CPR certification.
