Job summary

Aleca Hospice in Glendale, AZ is seeking a dedicated Registered Nurse to plan, organize and direct hospice care, emphasizing community health education and holistic patient support. You will handle in-home visits, develop care plans, administer medications, and communicate patient needs with the care team.

Candidates must have an active RN license, CPR certification, and a valid driver's license with insured vehicle.

Qualifications

• Must hold a current, valid RN license through the State Board of Nursing.
• Community health/hospice is preferred but willing to train the right candidate.
• Complies with accepted professional standards and practice.
• Demonstrates good verbal and written communication, and organization skills.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Possesses and maintains current CPR certification.

Responsibilities

• Completing hospice evaluations as requested (admission if comfortable and training would be provided).
• Conducting in home visits as assigned; assessing the patient’s condition, and teaching the patient and family.
• Developing care plans based on diagnosis and needs.
• Administering medications and treatments as prescribed by physician.
• Communicating patient needs and changes with the team as necessary.
