Crossroads Hospice & Palliative Care is seeking an Admissions Registered Nurse to ensure smooth transitions for patients and families. You will assess needs, provide direct patient care, and coordinate with the care team to develop individualized plans.

Qualificações

• 1+ years of skilled nursing experience preferred.
• Active RN license in state.
• CPR certification, TB results, and updated physical.

Responsabilidades

• Assess patient and family needs and share findings with case managers for care planning.
• Provide direct patient care: assess, monitor, administer medications, and perform treatments.
• Collaborate with the care team to develop individualized care plans for patients.
• Coordinate communication between patients, families and healthcare providers.
• Educate patients and families on disease management and self-care techniques.

Registered Nurse Admissions Responsibilities & Qualifications

• Associates, Bachelors or Masters Degree in Nursing accepted
• 1+ years of prior skilled Nursing experience is preferred
• An active, unencumbered state RN license
• Current driver's license, auto liability coverage and reliable personal transportation
• CPR certification, a copy of TB Results, and a copy of an updated physical
• Conduct timely and thorough assessments to evaluate patient and family needs, providing case managers with relevant information to ensure appropriate care planning and adjustments
• Provide direct patient care, including assessing and monitoring patients' conditions, administering medications, and performing treatments
• Collaborate with healthcare team members to develop and implement individualized care plans for patients
• Coordinate and facilitate communication between patients, families, and healthcare providers
• Educate patients and their families on disease management, treatment options, and self-care techniques
