Job summary

Vitality Hospice is seeking a dedicated Admissions Registered Nurse (RN) to join our clinician-owned hospice team. This full-time role involves on-the-road admissions, patient/family assessments, and close collaboration with the interdisciplinary team to ensure individualized care plans.

Monday through Friday, 10 AM–6 PM, with travel across assigned service areas. The position requires an active RN license and willingness to travel.

Qualifications

• Active RN license required.
• 1+ year skilled nursing experience preferred.
• Hospice or admissions experience preferred (not required).
• Willingness to travel within assigned service areas.
• Strong communication, organizational, and interpersonal skills.
• Valid driver's license, reliable transportation, and auto liability insurance.

Responsibilities

• Complete comprehensive hospice admissions and patient/family assessments
• Ensure accurate documentation and appropriate care planning
• Provide direct patient care as needed, including medication administration and treatments
• Collaborate with physicians and the interdisciplinary team to develop individualized plans of care
• Serve as a liaison between patients, families, and healthcare providers
• Educate patients and caregivers on disease progression, symptom management, and hospice services
