Vitality Hospice & Palliative Care is seeking an Admissions Registered Nurse (RN) to join our clinician-owned hospice team. This full-time role involves on-the-road admissions, patient/family assessments, and collaboration with an interdisciplinary team to plan individualized care.

Responsibilities include documenting assessments, patient care, medication administration, and educating families about disease progression and hospice services.

Qualifications

• Active RN license required.
• 1+ year of skilled nursing experience preferred.
• Hospice or admissions experience preferred (not required).
• Willingness to travel within assigned service areas.
• Strong communication, organizational, and interpersonal skills.

• Ensure accurate documentation and appropriate care planning.
• Provide direct patient care as needed, including medication administration and treatments.
