Job summary

Gentiva is seeking a compassionate Hospice Admissions RN to visit patients and families in homes, hospitals, or facilities, assess eligibility, and guide the admissions process with clinical confidence.

Applicants should hold an active RN license in the appropriate state, have 1+ year nursing experience, and possess CPR certification. A driving license and transportation are required to reach patients and coordinate with care teams.

Qualifications

• Hospice heart with patient-centered care
• Strong assessment and interpersonal skills
• Comfortable visiting patients in hospitals, SNFs, or home settings
• Valid driver's license and reliable transportation
• Current CPR certification

Responsibilities

• Visit patients and families in homes, hospitals, or facilities to assess hospice eligibility
• Explain hospice philosophy and services clearly and compassionately
• Guide patients through admissions process with supportive start to care
• Collaborate with physicians, case managers, and staff to coordinate transitions of care
• Document clinical findings and use nursing process for care planning
• Provide emotional reassurance to patients and families during decision-making
• Ensure compliance with state and federal regulations
• Support timely admissions and quality handoff with the care team

Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, you'll be the face of our care - helping families say "yes" to compassionate end-of-life support when it matters most.

What You'll Do as a Hospice RN Admissions Nurse:

• Serve as a key ambassador for our hospice services
• Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
• Explain hospice philosophy and services clearly, compassionately, and with clinical confidence
• Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
• Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
• Use the nursing process to assess patient needs, support care planning, and document clinical findings
• Build trust and provide emotional reassurance to patients and families as they make this important decision
• Accurately complete required documentation and ensure compliance with state and federal regulations
• Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff

About You

Qualifications - What You'll Bring:

• Active RN license in the state of employment (or eligible to obtain)
• 1+ year of general nursing experience
• A hospice heart: compassionate, empathetic, and deeply committed to patient-centered care
• Strong assessment, communication, and interpersonal skills
• Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
• Valid driver's license, auto insurance, and reliable transportation
• Current CPR certification

Preferred Experience (Not Required):

• 2+ years of hospice, home health, healthcare admissions, or medical sales
• Bachelor's Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification
• Background in case management, care coordination, or healthcare outreach
• Experience with electronic medical records and compliance documentation
