Job summary

Vitality Hospice & Palliative Care in the Bucks County area is seeking an Admissions Registered Nurse (RN) to join our clinician-owned hospice team. This full-time role involves travel, on-road responsibilities, and Monday–Friday scheduling, with a customer-focused approach to admissions.

You will conduct comprehensive admissions and patient/family assessments, document care plans, provide direct nursing care as needed, work with an interdisciplinary team, and educate families about disease

Qualifications

• Active RN license required.
• At least 1 year of skilled nursing experience.
• Hospice/admissions experience preferred but not required.
• Willingness to travel within assigned service areas.
• Strong communication, organizational, and interpersonal skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete comprehensive hospice admissions and patient/family assessments.
• Ensure accurate documentation and appropriate care planning.
• Provide direct patient care as needed, including medication administration and treatments.
• Collaborate with physicians and the interdisciplinary team to develop individualized plans of care.
• Serve as a liaison between patients, families, and healthcare providers.
• Educate patients and caregivers on disease progression, symptom management, and hospice services.
