Job summary

Crossroads Hospice & Palliative Care is seeking Registered Nurse Admissions to support smooth patient transitions, with compassion and efficiency. You will be the first contact for education and reassurance, working with RN Case Managers to ensure effective communication of patient and family needs.

The role emphasizes thorough assessments, direct patient care, and collaboration with the care team to develop individualized plans and educate families on disease management.

Qualifications

• Active, unencumbered RN license in the state.
• Current driver's license, auto liability coverage and reliable personal transportation.
• CPR certification, TB results copy, and updated physical.

Responsibilities

• Conduct timely and thorough assessments to evaluate patient and family needs; provide information for care planning.
• Provide direct patient care, including assessing and monitoring patients' conditions, administering medications, and performing treatments.
• Collaborate with healthcare team to develop and implement individualized care plans.
• Coordinate and facilitate communication between patients, families, and healthcare providers.
• Educate patients and their families on disease management, treatment options, and self-care techniques.

They also serve as a critical link to the care team, including Registered Nurse Case Managers, ensuring that patient and family needs are effectively communicated.

Registered Nurse Admissions Responsibilities & Qualifications:

• Associates, Bachelors or Masters Degree in Nursing accepted
• 1+ years of prior skilled Nursing experience is preferred.
• An active, unencumbered state RN license.
• Current driver's license, auto liability coverage and reliable personal transportation.
• CPR certification, a copy of TB Results, and a copy of an updated physical.
• Conduct timely and thorough assessments to evaluate patient and family needs, providing case managers with relevant information to ensure appropriate care planning and adjustments.
• Provide direct patient care, including assessing and monitoring patients' conditions, administering medications, and performing treatments.
• Collaborate with healthcare team members to develop and implement individualized care plans for patients.
• Coordinate and facilitate communication between patients, families, and healthcare providers.
• Educate patients and their families on disease management, treatment options, and self-care techniques.

• Schedule:Tuesday through Friday 10:00 AM to 8:30 PM
• No mandatory on-call required
