You will deliver patient-centered nursing care, assess residents' health, and collaborate with physicians and therapists to create individualized care plans. The role requires a valid RN license, strong geriatric skills, and current BLS/CPR certification.

Responsibilities

• Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting.
• Assess, monitor, and document residents' health conditions and progress.
• Administer medications and treatments as prescribed, ensuring facility protocols.
• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
• Provide education and support to residents and families regarding care plans and health conditions.
• Maintain accurate and timely documentation in compliance with state and federal regulations.
• Foster a safe, comfortable, and therapeutic environment for residents.
• Supervise and mentor nursing assistants to ensure quality care delivery.

Position Summary: Registered Nurse ( RN )

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team. The RN will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with the clinical team, physicians, and other healthcare professionals to ensure the highest standards of care in our long-term care and rehabilitation facility.

Qualifications:

1. Valid Registered Nurse (RN) license
