PRN should actually feel flexible.
As a Skilled Home Health RN, you will:
Perform comprehensive skilled nursing assessments
Complete Start of Care, Resumption of Care, Recertification, and Discharge visits
Complete OASIS assessments as required
Develop and update individualized plans of care
Provide wound care and monitor wound progression
Provide medication management and reconciliation
Monitor patients for changes in condition
Provide disease-management education
Educate patients, families, and caregivers
Communicate with physicians regarding changes in patient status and orders
Coordinate care with PT, OT, SLP, MSW, and other members of the interdisciplinary team
Help identify concerns early to reduce avoidable hospitalizations
Complete accurate and timely documentation using WellSky
Ensure care is provided according to physician orders, agency policies, and applicable regulations
Active Washington, DC Registered Nurse License Required
Graduate of an accredited nursing program
Strong nursing assessment and clinical decision-making skills
Reliable transportation and ability to travel between patient homes
Excellent communication, documentation, and organizational skills
Ability to work independently in the field
Comfortable managing skilled home health patients
Home Health and OASIS Experience Preferred
