Job summary

Human Touch Home Health is seeking compassionate Registered Nurses (RNs) for PRN home health opportunities in Chantilly, Centreville, Fairfax, and surrounding areas. Build your caseload around your availability and choose the areas you want to cover.

You will perform skilled nursing visits, assessments, Start of Care and Discharge visits, wound care, and patient education while coordinating with the care team and documenting with WellSky.

Qualifications

• Graduate of an accredited nursing program.
• Current CPR/BLS Certification.
• Active Virginia RN License.
• Ability to work independently in patients' homes.

Responsibilities

• Complete skilled nursing visits in patients' homes.
• Perform comprehensive nursing assessments.
• Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned.
• Provide wound care, medication management, and disease education.
• Monitor changes in patient condition.
• Educate patients and caregivers.
• Coordinate care with physicians, therapists, and other members of the care team.
• Complete accurate and timely documentation using WellSky.

Patient opportunities may include Chantilly, Centreville, Fairfax, Fair Oaks, Oakton, Burke, Clifton, and surrounding communities.

You do NOT have to cover the entire territory. Tell us where you want to work, and we'll match opportunities based on patient needs and your availability.

What You'll Do

What We're Looking For

• Active Virginia Registered Nurse (RN) License

• Graduate of an accredited nursing program

• Current CPR/BLS Certification

• Valid Driver's License and reliable transportation

• Strong clinical assessment and communication skills

• Ability to work independently in patients' homes

• Home Health and OASIS Experience Preferred, but Not Required

• WellSky Experience is a Plus
