Job summary

Human Touch Home Health is seeking Virginia-licensed RNs for PRN home health visits in Alexandria, Springfield, and surrounding Northern Virginia communities. You’ll select areas to cover, build your caseload around availability, and deliver one-on-one skilled nursing care in patients’ homes.

Responsibilities include Start of Care, Recertifications, care planning, wound care, med management, and patient education, with WellSky documentation and strong clinical leadership support.

Qualifications

• Active Virginia RN license, in good standing.
• Graduate of an accredited nursing program with current CPR/BLS.
• Home health experience or Oasis knowledge is preferred but not required.
• Valid driver’s license and reliable transportation.
• Ability to work independently in a home setting and manage caseloads.

Responsibilities

• Perform skilled nursing visits in patients' homes.
• Conduct comprehensive nursing assessments and start/recertify care plans.
• Provide wound care, medication management, and disease education.
• Monitor patient condition and coordinate with physicians and therapists.
• Document visits accurately using WellSky in a timely manner.
• Educate patients and caregivers to support safe, independent living.

Your Territory

Patient opportunities may include:

Alexandria, Springfield, Annandale, Burke, Lorton, Newington, Falls Church, and surrounding areas.

You do NOT have to cover the entire territory. Let us know where you want to work.

What You'll Do

• Complete skilled nursing visits in patients' homes
• Perform comprehensive nursing assessments
• Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
• Provide wound care, medication management, and disease education
• Monitor changes in patient condition
• Educate patients and caregivers
• Coordinate care with physicians, therapists, and other members of the care team
• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Registered Nurse (RN) License
• Graduate of an accredited nursing program
• Current CPR/BLS Certification
• Valid Driver's License and reliable transportation
• Strong assessment and communication skills
• Ability to work independently in a home health setting
• Home Health and OASIS Experience Preferred, but Not Required
• WellSky Experience is a Plus
