Job summary

Human Touch Home Health is seeking compassionate Registered Nurses for PRN home health opportunities across Chantilly, Centreville, Fairfax and surrounding communities. Choose your territory and build a caseload around your availability, with flexible scheduling, mileage reimbursement and one-on-one patient care.

All roles involve skilled nursing visits, care planning and coordination with the care team. Active Virginia RN license, CPR/BLS, and ability to work independently in patients' homes

Qualifications

• Active Virginia RN license
• Graduate of an accredited nursing program
• Current CPR/BLS certification
• Valid driver''s license and reliable transportation
• Ability to work independently in patients'' homes
• Home Health and OASIS experience preferred, but not required
• WellSky experience is a plus

Responsibilities

• Complete skilled nursing visits in patients' homes
• Perform comprehensive nursing assessments
• Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
• Provide wound care, medication management, and disease education
• Monitor changes in patient condition
• Educate patients and caregivers
• Coordinate care with physicians, therapists, and other members of the care team
• Complete accurate and timely documentation using WellSky

Patient opportunities may include Chantilly, Centreville, Fairfax, Fair Oaks, Oakton, Burke, Clifton, and surrounding communities.

You do NOT have to cover the entire territory. Tell us where you want to work, and we'll match opportunities based on patient needs and your availability.

What You'll Do
