Human Touch Home Health is actively hiring Registered Nurses (RNs) for Full-Time, Part-Time, and PRN skilled home health opportunities throughout Upper Marlboro and Southern Prince George’s County.

Qualifications

• Maryland RN license required.
• Graduate of an accredited nursing program.
• Reliable transportation and valid driver’s license.

Responsibilities

• Perform comprehensive skilled nursing assessments.
• Complete Start of Care, Resumption of Care, Recertification, and Discharge visits as assigned.
• Complete OASIS assessments as required.
• Develop and update individualized plans of care.
• Provide wound care and other skilled nursing treatments.
• Perform medication reconciliation and provide education.
• Coordinate care with the interdisciplinary home health team.
• Document accurately using WellSky.

What You’ll Do

• Perform comprehensive skilled nursing assessments
• Complete Start of Care, Resumption of Care, Recertification, and Discharge visits as assigned
• Complete OASIS assessments as required
• Develop and update individualized plans of care
• Provide wound care and other skilled nursing treatments
• Perform medication reconciliation and provide medication education
• Provide disease-management education
• Monitor patients for changes in condition
• Communicate with physicians and obtain necessary orders
• Help identify concerns early and reduce avoidable hospitalizations
• Educate patients, families, and caregivers
• Coordinate care with the interdisciplinary home health team
• Complete accurate and timely clinical documentation using WellSky

What We’re Looking For

• Active Maryland Registered Nurse License Required
• Graduate of an accredited nursing program
• Reliable transportation
• Strong clinical assessment and critical-thinking skills
• Excellent communication and documentation abilities
• Ability to work independently in patients’ homes
• Home Health and OASIS Experience Preferred, but Not Required
• WellSky Experience is a Plus
