Job summary

Phoenix Home Care & Hospice is seeking an RN for Private Duty Care and VA departments to cover in-home visits. You will oversee aides and perform routine nursing visits, including medication setups and maintaining the plan of care. Services may be privately paid, funded by long-term care insurance, or VA.

Qualifications

• Active MO RN license required.
• In-home care experience preferred.
• Valid driver’s license required.
• Excellent communication and compassion for patients.

Responsibilities

• Provide basic patient care: vitals, weekly medication management per physician orders, monthly diabetic nails, monthly skin monitoring, and bi-annual general health assessments.
• Open new cases and obtain related consents and documentation per policy.
• Observe, record, and report responses to treatment and changes in patient condition to appropriate personnel or physician.
• May supervise In-Home Aides for Advanced Personal Care (APC) services.
• Provide instruction/education on medications when needed.
• Follow state approved care plans.
• Attend quarterly nurse meetings.
• Demonstrate good time management and attention to detail, be a self-starter.

Responsibilities:

Requirements:
